Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this website. Also, take some time to review our staff page and familiarize yourself with the doctors. We look forward to your first visit.

First Appointment Checklist

  • Visited Patient Portal
  • Completed Paperwork
  • Medication List (We will be happy to photocopy it for your file)
  • Photo ID
  • Insurance Card
  • Form of Payment (Please always be prepared to pay at the time of service by having your wallet with you at every visit)
  • Parent or Guardian if you are under 18-years of age
  • We recommend all new patients verify for themselves that we are in-network; to do this go to your insurance provider’s website and search in-network physicians or call the customer service number on your card

Referral?

Were you referred to our office with one of our referral cards (shown below)? Bring it with you to your first appointment and you will receive a special welcome gift. We value the referrals we receive from current patients, area doctors, our community and employees.
